Each payor source has specific guidelines that they require; however, there are things  you should find helpful that can be adapted or supportive accordingly.

1. Understanding the value of clear, consistent documentation standards  
2. Identifying required assessment criteria, including common areas of deficiencies and how to address these  
3. Identifying required treatment plans, common deficiencies in treatment planning and resources to address deficiencies  
4. Understanding the requirements for documenting progress notes and how they align with the road map for treatment
